I will be in Spokane end of next week on a short trip and may have time to take in a cigar. Wondering if anyone has a recommendation as to where to grab a drink and a smoke that would have enough ventilation that the lady friend wouldn't get smoked out.

No smoking in-doors at the bars or cigar shops in Washington state.   As far as Spokane locations, I've visited Cigar Train at 1010 W Rosewood Ave, Spokane, WA 99208 which had a very friendly proprietor and decent but not great selection.   The only exception to the indoor smoking ban are the casinos on Indian land, specifically Northern Quest Casino's Legend's of Fire Lounge, a full service cocktail lounge with a walk-in humidor.  Expect Washington cigar prices to be 30% over MSRP.
