I've been without a cutter a couple of times. If I have a ball-point pen handy (the kind with a clicker on top), I un-screw the pen barrel, and use the male threaded end as a punch. It's kind of shaped like a punch...just have to be careful because, no kidding, they're usually quite dull. Gets the job done though.
I've also used a box cutter. Didn't slice the cap, I used the corner of the cutter to cut a circle in the cap and then pinched off the part I cut out.
Also you can use a large nail as a punch. Don't use a screw though, it'll grab the wrapper and tear the crap out of it.
